How I think about this
Product problem
Care teams lose visibility into patient health between visits. Signals from check-ins, symptom reports, and care companion interactions accumulate without a structured workflow to review, prioritise, or act on them.
User / system workflow
Incoming patient signals are classified by risk level and surfaced in a prioritised queue. Providers review alerts, update care status, escalate high-risk cases, and document decisions — all within a single, traceable console.
My product lens
The design problem is not just alerting — it is making alerts reviewable and actionable without adding friction to already-stretched care teams. Every element of this console is structured around provider workflow, not data completeness.
Why it matters
Chronic condition management only works if care teams can act on what they know between visits. A governed signal-to-action layer closes the loop between AI outputs and real care-team decisions.
What I would improve next
Workload distribution tools for multi-provider teams; integration surface for EHR task creation; risk trend visualisation across the patient panel.
